For older adults encountering the challenges of aging, or for children of those adults, our care managers help navigate the complex maze of health care
options as well as other aging-related difficulties. We do everything from helping you with bureaucracy (say, get a new social security card), to improving communication with
health care providers and other social service organizations, to figuring out which rehabilitation facility best fits your budget and needs.

For those having trouble meeting basic needs, our care managers at the Barbash Family Vital Support Center help you with everything from getting food from the
food pantry, to budgeting, coaching and encouragement in life skills, to supportive, practical counseling (providing support to those facing emotional, behavioral, social, or
career-related challenges), and more.

For Holocaust survivors, our care managers—depending
on need and eligibility—regularly
visit your home for companionship, help with hearing exams, help with translation (say, at a doctor's office), help with transportation, and more.

For mental health care, mental health care managers works alongside you to coordinate referrals and resources to ensure you or your loved one receive continuous,
person-centered, and effective support for your well-being.
