Care management is an umbrella term that means, at its core, we help you manage your life to live with independence and dignity.
We do this in different ways, depending on your needs or your loved one's needs.
- At our Barbash Family Vital Support Center, our care managers help clients with everything from getting food from the food pantry, to budgeting, to being an encouraging life coach, to
counseling; ie, providing guidance and support to those facing emotional, behavioral, social, or career-related challenges.
- For Holocaust survivors and Russian-speaking older adults, our care managers can, depending on need and client qualifications, do regular home visits for companionship, help with hearing
exams, help with translation (say, at a doctor's office), help with transportation, and more.
- For older adults who need help figuring out next steps as they encounter challenges as they age, or for children of those adults, our care managers help navigate the complex maze of health
care options. We do everything from helping you get a new social security card, to figuring out which rehab facility best fits your budget and needs, to how to get free transportation
services from the state. We are experts at older adult care, and our job is to help you.
A care manager will evaluate an individual’s unique needs, plan strategies to address those needs, keep family members and health teams informed, and be a steady, informed resource who
coordinates and tracks what's necessary.
